One owner per item
If an action item needs two owners, it usually means it should be split into two items.
Track every action item with an owner and due date, filter by status, and let overdue items highlight themselves automatically.

Giving every item a named owner and a specific due date, then reviewing the list regularly, is a simple way to keep that follow-through visible instead of relying on memory. The overdue highlight is deliberately unforgiving, since a quietly overdue item is exactly the kind of thing a tracker should surface.
