A 5 mm gap is the familiar standard for many bullet journals. On screen, that usually feels similar to a 19 or 20 pixel spacing at standard CSS resolution, which is why the Journal preset starts at 20 pixels. Use 12 to 16 pixels for dense technical diagrams, 20 to 28 pixels for notes and planning, or 32 pixels and above for loose sketching and presentation backgrounds.
Printers can slightly scale a page. For the most predictable spacing, download the A4 or US Letter SVG and print at 100% or “actual size” rather than “fit to page.”
